You must bring the following documents to Team Check-in:
If you ARE A MEMBER of US Clubs you will need to bring:
1. US Club Soccer Player cards.
2. US Club Soccer medical releases.
3. Club Soccer Tournament Roster
4. Your Hotel information if you are staying over (hotel name, # of rooms, check in and check out dates, hotel rate $)
5. Emergency cell phone #’s for your team in case of rain, etc.
If you ARE NOT A MEMBER of US Clubs you will need to bring: THIS INCLUDES ALL SAN DIEGO AND CYSA-S TEAMS
1. Approved travel papers from your State Association. PLEASE DOUBLE CHECK THAT YOUR STATE OFFER TRAVEL INSURANCE ALONG WITH THE TRAVEL PAPERS i.e., Colorado, Cal North and Utah some times do not.
2. Approved travel roster from your State Association.
3. State Player Cards.
4. Medical Releases.
5. Your Hotel information if you are staying over (hotel name, # of rooms, check in and check out dates, hotel rate $)
6. Emergency cell phone #’s for your team in case of rain, etc.
If you are unable to obtain travel papers from your State Association you will be able to purchase Tournament Insurance at Team Check-in.
IF OUR STATE HAS REFUSED TO GIVE YOUR TEAM TRAVEL PAPERS Including travel insurance
Please note some states offer travel permission but not insurance. Colorado for example. To purchase temporary travel insurance, please contact the US Clubs representative below or visit their web site at www.usclubsoccer.org
