(Note: If player does not have a US Birth Certificate he will be required to show proof that he has been in the United States before the age of 12.  This can be done by providing copies of school reports, or transcripts, or immunization records from a physician, these documents will need to be turned in with registration paperwork and birth certificate in order to obtain international clearance).
